Welcoming Our New Hospital Liaison and Business Development Personnel

We are so pleased to welcome Sandy Kaczur, Hospital Liaison and Business Development Personnel to our Foundations Health Solutions team! Sandy brings 15 years of Skilled Nursing Admissions experience for most of Northeast Ohio. With her enthusiastic, caring and driven personality, she enjoys helping to make it a smooth transition into post-acute care for patients, families and referral sources in what could be a difficult time.



While not at home, Sandy enjoys family time with her husband and son in Brunswick, along with taking mini getaways with family and friends.

"It can be so rewarding to educate, guide and help all who run in the path of transitioning to Skilled Nursing or Long-Term Care. At the end of the day I can go home and say that I do love what I do and that maybe, just maybe, I made a difference today."
